- Appointment Bookings and Cancellations
- Accuracy: When booking appointments online, you agree to provide accurate, current, and complete information about yourself.
- Cancellation Policy: Active Pulse Physiotherapy reserves the right to enforce a cancellation policy. If you need to cancel or reschedule an appointment, you must provide notice within our specified timeframe (typically 24 to 48 hours). Failure to do so may result in a cancellation fee, which is your sole financial responsibility and cannot be billed to insurance.
- Direct Billing and Insurance Acknowledgement
Active Pulse Physiotherapy offers direct billing to most major insurance providers as a convenience to our patients.
- No Guarantee of Coverage: While we facilitate direct billing, we cannot guarantee that your specific insurance plan will cover the full cost of your treatment (Physiotherapy, Chiropractic Care, Massage Therapy, etc.).
- Patient Responsibility: You are ultimately responsible for understanding your own insurance policy parameters, coverage limits, and deductibles. If your insurance company denies a claim, pays less than expected, or if your coverage maxes out, you remain fully responsible for the outstanding balance owed to Active Pulse Physiotherapy.
- Permitted Use of the Site
You are granted a limited, non-exclusive, non-transferable, and revocable license to access and use the Site strictly for personal, non-commercial purposes.
You agree not to:
- Use the Site in any way that violates federal, provincial, or local laws and regulations.
